Some aftermarket axles have shoddy clips. Also a chance they re-used the original clip if the axle didn't come with one which would cause this issue. If it happens again don't let them sell you a new axle just get a new clip!

Sorry i just saw this thread. i would have told you that it was axles. They're notorious for breaking. I have a buddy that can swap axles in a Civic from wheels on the ground to wheels on the ground is 20 minutes...not kidding. I watched him do it once. Glad you got it fixed.
