Paved or Dirt road?
Anyone ridden this route before?

That road has one short stretch that is gravel. We typically opt to turn onto Bald Mountain Road - narrow single lane road but entirely paved. There may be some debris still, so ride it slow the first time. I edited your map to show you the difference.
When coming from Carlton, it is a road that turns off to the left (sharply) and starts uphill. Basically, it is the only paved road you will come to once you are out there...

If you come to gravel you have gone too far. Once on Bald Mountain Road, you will ride for a few miles before coming to a Y in the road... that is Gilbert Creek/Bible Creek. Turn right takes you back down to Nestucca, Turn Left takes you into Willamina.

I rode this route last year out to sand lake. IIRC there was a 1.5 mile stretch of gravel, gravel & old pavement with some moss for decoration. It was really easy as long as I kept it at < 20mph. I was worried more about the potholes than the gravel. I never did see the turnoff that others talk about but I'm glad I never did because I never would have had this cool pic

BTW, it was a BLAST!

Bill is spot on. I was up there three weeks ago riding from Carlton west and didn't make it more than 8-9 miles before I was up in the snow. I didn't even make it to the first turnoff. As of then, that road needed some serious cleanup.

looks like the road i had my first crash on.... ah memories. it's pretty clean if you stay on the paved path (have no idea what it's called, i was following a friend) but it's alot of single lane road, and their is crap on it in places, especially the mid corners under tree's..... moss pile f#*ckin sh%t

That road kicks ass if ya like the tight stuff. There were some rough sections of pavement on the lower portion of the west side when I hit it last year.

I prefer the lower route with the dirt road section (go figure). It's only about 3 miles long. I've ridden the upper route too and was surprised by dirt bikes crossing the road in front of us from one trail to another...at speed, and by ATV riders on the lane-and-a-half wide pavement making like it was their own private speedway. Drazen (DGA) rode his Triumph with us the last time I did the dirt portion and had no problem with it. Much safer of the two routes IMHO.
